Change Request Form

This request form can be used for:

• Moving a player from one section to another.
• Moving a player from one school team to a different school team (if you accidentally selected the wrong school).
• Changing a player’s school grade (if entered incorrectly).
• Updating a player’s BYE option (BYE = request not to play in certain round(s)).

Steps:

• 1. Only the email address used for registration can submit this request.
• 2. If you are a coach or coordinator, please ask the parents or the person who registered to complete the form.
• 3. Fill out this request form and submit it.
• 4. The system will send a confirmation email to the registered address.
• 5. Open the email and click the link to confirm your request.
• 6. Double-check the pre-entries to ensure everything is correct. If you encounter any issues, please use the contact page.
